The Patient Services Team have the difficult job of helping both patients and doctors. Occasionally the reception area becomes extremely busy with telephone calls, requests for visits and patients attending or making appointments. Please try to be considerate if you have to wait - your needs will be met as soon as practicable.

Our Patient Services Team can help you with the following:

Blood Test/X-Ray Results
Holiday Cancellation Forms
Hospital Admission Forms
Insurance Forms
Private Medicals
Passports
Referrals to Chiropody
Requests for Hospital Transport
Sick Certificates
Sickness/BUPA Claim Forms

Please note that sick notes will take two working days and non-NHS forms will take five working days before they are ready for collection from reception. There may be a charge for completing non-NHS forms.

If you wish to discuss with the doctor the results of tests or a letter from the hospital please check beforehand with Patient Services that we have received the necessary information.

We realise that our reception desk is very open. If you wish to talk to the receptionist confidentially please say so and we will be pleased to take you to a private room and help you there.

Our Patient Services staff can take messages to pass on to the doctors or other members of staff, can check if your hospital referral or letter has been typed and can arrange to change a referral from NHS to private. Our Practice Secretary can also give you the relevant telephone numbers if you wish to chase a hospital appointment or enquire about private consultations.
